Frenchtown Medical is happy to offer online scheduling for you and our community. We require our patients to check in 15 minutes prior to the scheduled appointment with a photo ID, insurance card or form of payment at the time of service.

You must be 18+ years to schedule and will need a parent or guardian to be present at your appointment.

Our office may need to reach out to you for more information regarding you or your appointment, please list your phone number when prompted.

All new patients that wish to establish Primary Care at Frenchtown Medical will need to complete a New Patient Packet.

If you need to cancel your appointment prior to its start time, please call the practice in advance. Per our cancellation policy, a late cancellation or no show may result in a fee of $50.
